This waiver signifies that I,the undersigned, acknowledge and agree that:

1. The activities involved in this program present a risk of injury that may be significant, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of injury does exist;

2. I KNOWINGLY ASSUME ALL SUCH RISK, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation;

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of It’s A Zoo Life immediately;

4. I HEREBY RELEASE RESPONSIBILITY for It’s a Zoo Life, any and all agents and/ or employees, other participants, sponsoring agencies, advertisers, and if applicable, owners and lessors of premises used for the activities, WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, LOSS, OR DAMAGE to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

5. I hereby grant It’s a Zoo Life permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of the It’s a Zoo Life and will not be returned.

I HAVE READ THIS RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY.

 

 

FOR PARENTS/GUARDIANS OF PARTICIPANTS UNDER AGE 18. This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above. I release It’s A Zoo Life from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.
